Activity Application Form Add On

The Activity Application Form add-on offers a convenient way of collecting, reviewing, and managing submissions for activity applications. When the activity is approved, it can easily converted to a course with pre-populated data fields.

This add-on comes with two additional roles; CME manager and CME reviewer. Each role has its own Activity Application Dashboard.

CME manager - Can view and edit all Activity Application Form submissions.

CME reviewer - Can view and edit all Activity Application Form submissions assigned them.  The CME reviewer can also create a course using the Activity Application Form.
