  1. Log in as a user with the site admin role.
  2. Click the wrench to open the admin menu.
  3. Click "Administration." 
  4. Click "Structure."
  5. Click "Faculty Relationship."
  6. Click "+ Add bundle" in the top navigation bar.
  7. Type the form name into the "Type" field.
  8. Click "Save."
  9. Proceed to adding fields to the new form.
