Faculty forms are intended to be used across multiple courses. It is not recommended to create per-course forms.
To create a new faculty form:
- Log in as a user with the site admin role.
- Click the wrench to open the admin menu.
- Click "Administration."
- Click the "+" sign next to "Structure."
- Click the "+" sign next to "Entity types."
- Click "Faculty Relationship."
- Click "+ Add bundle" in the top navigation bar.
- Type the form name into the "Type" field.
- Click "Save."
Proceed to adding fields to the new form.
