Completing Faculty Forms

Completing Faculty Forms

After faculty are added to a course they will receive an email notification asking them to create an account or log in.

After logging in, the faculty will see a message that they have outstanding faculty forms.

Clicking "Complete form" will take the faculty to the form and allow them to submit it.

If faculty have not yet completed a baseline disclosure, they will be required to complete it before submitting any forms.
