Webforms

A Webform is an online form that can collect ungraded information from authenticated users. Webforms are highly flexible and can be used in a variety of contexts across your EthosCE site.


Common Uses for Webforms

  • End-of-course evaluations — Collect learner feedback after completing a course to assess quality, relevance, and overall satisfaction.

  • Post-assessment surveys — Gather responses after a quiz or exam to understand the learner experience.

  • General surveys — Poll your audience on topics relevant to your organization or continuing education programs.

  • Contact / inquiry forms — Provide a way for users to reach out with questions or requests without needing to email directly.

  • Needs assessments — Gather data from your audience to help shape future educational offerings.


Creating a Webform

Step 1: Create the Webform

  1. From the top navigation bar, click + New.

  2. In the dropdown menu that appears, scroll to the bottom of the list and click Webform.

  3. On the Create Webform page, enter a Title for your webform.

  4. Optionally, select an existing webform from the Available webforms dropdown under Webform Template to copy its fields as a starting point.

  5. Click Save at the bottom right of the page.

Step 2: Build Your Form

After saving, you will be taken to the Webform tab where you can begin building your form. This tab contains four sub-sections:

  • Form components — Add, arrange, and manage the fields that make up your form. Enter a name for your new component, select a field type (e.g., Textfield, Select, Grid), and click Add to include it. Fields you add will appear in the components list with their label, form key, type, and required status.

  • Conditionals — Set up logic rules to show or hide fields based on a learner's previous answers.

  • E-mails — Configure automatic email notifications to be sent when a form is submitted, either to an administrator or to the learner.

  • Form settings — Control submission behavior, confirmation messages, access permissions, and other form-level options.

Tip: You can return to any of these sub-sections at any time by navigating to the webform and clicking the Webform tab.
